Intake 

Please download and save pdfs to your computer to complete and submit the intake form via email to info@braydensupervision.com or by fax to 416-245-4116.

Both parties are required to apply for supervised parenting time services by submitting completed Intake forms. The Intake fee is processed once the required paperwork is received from both parties. Your assigned Service Director will reach out to complete the Intake process and schedule supervision for the parenting time visits. This process typically takes 5–7 business days to complete, provided there are no issues and both parties cooperate.
